Patient Support Coordinator
The Vasculitis Foundation (VF) is the international organization for patients with vasculitis, their families, friends, and the health care professionals who care for them. The VF advocates for early diagnosis and leading-edge treatment for all patients, no matter where they live. The VF partners with researchers around the world to determine the cause and discover the cure for vasculitis.
The Patient Support Coordinator is a vital role in support of the mission and vision of the VF and serves as the first person of contact for patients, their family members, care partners, and health care providers.
Working closely with the VF Executive Director and other members of the VF staff, the Patient Support Coordinator is responsible for providing phone and email support directing families to the resources and information they need. This role requires empathy and compassion and the ability to assist people in navigating the complexities of a diagnosis of a rare disease, often in stressful situations.
Responsibilities

Answer phone calls, voicemails, and email requests in timely and professional manner

Provide information on VF programs, services, and resources

Perform online research to answer questions and provide basic medical information, resources, and services to assist patients and their families in their quest for help

Liaison with the VF Support Group Moderator to organize and maintain the weekly and monthly support groups; attend support group meetings to meet members of the community

Collaborate with VF staff and Moderator to develop training materials for new VF Ambassador Program

Connect patients and care partners with other members of the vasculitis community to provide moral support and basic information

Follow up with newly diagnosed patients and their care partners as time permits

Monitor VF’s social media platforms (Facebook, Instagram, LinkedIn, and Twitter) daily to identify any requests/comments that may need a response

Mail informational materials to patients as needed

Perform daily data entry in the VF Salesforce database

Maintain and update the Find a Doctor Excel spreadsheet on a monthly basis
What you need

Ability to learn about vasculitis, VF services, programs and resources
-
Ability to work independently and multi-task, prioritizing the most important tasks

Ability to maintain confidentiality

Ability to meet and serve the public by phone and email and occasionally in person at regional and national conferences

Ability to effectively communicate with a diverse range of people of all ages, educational, geographical regions, and socioeconomic backgrounds

Excellent verbal, written, and data-visualization communication skills
-
Excellent organizational skills and attention to detail

Proficient with Microsoft Office Suite, Google Drive, Salesforce, and WordPress
Minimum qualifications include a bachelor’s degree and experience or can be met by an equivalent combination of education, work experience and life experiences that can provide the skills needed to perform the job.
Physical and Environmental Conditions

Physical ability to bend, reach and lift moderate loads (25+ lbs.);

Ability to independently travel locally, regionally, and nationally as required.

Work environment involves everyday risks or discomforts that require normal safety precautions typical of such places as offices or meeting rooms, e.g., use of safe work place practices with office equipment, and/or avoidance of trips and falls
