Thrive Health Connection (formerly Good Samaritan Project) is the original AIDS service organization addressing HIV/AIDS in the KC Metro since 1984. The organization recently underwent an exciting transformation, reinvigorating its brand while continuing to provide high-quality free HIV/STI testing and education for the community and case management for our customers. Thrive Health has some of the highest viral suppression rates in the Transitional Grant Area at 93%.

Position Description:   Provide trauma informed counseling and care coordination to uninsured clients who meet eligibility requirements at Thrive Health Connection and throughout the Kansas City Transitional Grant Area (KCTGA). Work cooperatively with Kansas City Missouri Health Department and other sub-recipients to administer mental health services and serve as a point of referral for individuals regardless of their eligibility.
Supervisor:  Director of Programs
Work Status:   0.5 FTE with potential for 1.0 FTE if additional funding secured.
  • Provide individual and group counseling to eligible individuals.
  • Provide crisis intervention and support to all individuals regardless of eligibility.
  • Participate in routine clinical supervision with Thrive Health Connection contracted clinician.
  • Complete home and community visits with individuals.
  • Work collaboratively with KCHD and other sub-recipients in the KCTGA
  • Collect and enter data into statewide client level database including accepting and processing referrals.
  • Maintain records as outlined by contract.
  • Complete administrative tasks as outlined by contract.
  • Assess eligibility and ensure Ryan White is payor of last resort when assisting individuals.
  • Promote increased enrollment into mental health program.
  • Develop policy for following clients that do not connect to care when referred.
  • Monitor care and implement corrective action plans as needed.
  • Participate in Service Excellence (Quality Improvement) efforts at Thrive Health Connection and throughout KCTGA.
  • Perform other duties as assigned.
  • Licensed Mental Health Clinician, preferably LCSW or LPC.
  • Fluency in Spanish as a second language strongly preferred.
  • Knowledge of LGBTQ+ community preferred.
  • Knowledge of HIV and STI risk behaviors, human sexuality, and social determinants of health.
