Bilingual Medical Case Manager
The Medical Case Manager assists individuals living with HIV with issues as they relate to HIV/AIDS in acquiring medical care, adherence counseling in terms of medical appointments and medications, and accessing social service and/or other community resources for continuity of care. Case Managers assist clients through advocacy, resource linkage, and supportive counseling. Case Managers work to improve the dignity and quality of life of those affected by HIV/AIDS through education, confrontation of systemic barriers, and promotion of mental, physical, emotional, and social empowerment of mental, physical, emotional, and social empowerment. The goal of Case Management is to provide culturally sensitive, trauma-informed care to better enable clients to become independent and self-sufficient. Additionally, Case Managers will fulfill the requirements of grant(s) for which they are funded.
Scope of Responsibilities
Vivent Health is one of the nation’s largest providers of HIV prevention, care and treatment services with a budget approaching $150 million and employing over 450 professionals.
The unique, nationally recognized Vivent Health HIV Medical home model of care assures that everyone with HIV has access to medical, dental, mental health and social services and provides the best opportunity for patients to achieve high quality health outcomes. Vivent Health also provides aggressive HIV prevention services to gay men, injection drug users and others at the highest risk for HIV infection.
Vivent Health currently operates in multiple states nationwide. It embraces a Market Culture to increase access to care, enhance quality outcomes, achieve and surpass financial performance expectations and has an aggressive plan to expand into additional states.
- Conducts a comprehensive biopsychosocial culturally sensitive assessment with the client and/or family/support network.
- Develops with the client and/or family a mutually agreed upon plan of care to meet the needs of the client as identified in the assessment. The plan must be action-oriented and time-specific.
- Arranges and coordinates multiple services for the client and/or family. Provide ongoing monitoring of the activities implemented to carryout the plan of care.
- Evaluates the effectiveness of the plan of care in achieving the desired outcomes with clients and/or families, the treatment team and external service providers.
- Maintains current knowledge of medical and psychosocial developments concerning AIDS and related conditions.
- Educates the client and/or family about the medical and psychosocial aspects of HIV/AIDS.
- Attend medical appointments with clients as needed and when requested by clients
- Provides supportive counseling to the client and/or family relative to HIVAIDS.
- Negotiates and advocates for the development and delivery of needed services with internal and external service providers.
- Assists in developing and planning community, region, or statewide client services; participates in monitoring such services to ensure proper service delivery.
- Collaborates as team member to achieve organizational and department goals.
- Maintains manual and computerized documentation in the client record
- Performs other duties as assigned.
- BSW/MSW/RN or possess a college degree in a related field with two years case management experience.
- Experience working with individuals of diverse cultures, ethnicities, socioeconomic backgrounds, sexual orientations, and gender identities and/or expressions preferred
- Bilingual (English and Spanish) skills
- Knowledge of HIV, comorbidities, mental health, substance abuse, or other infectious diseases
- Ability to communicate effectively, both orally and in writing
- Ability to prioritize multiple tasks and competing priorities
- Ability to establish and maintain good working relationships with coworkers
- Ability to demonstrate good judgement, maintain strict confidentially standards, and adhere to professional standards as defined by state and federal regulations
- Ability to make sound decisions in accordance with agency policies, procedures, and guidelines
- Ability to work comfortably with diverse populations, with sensitivity to issues concerning HIV and all disabilities
- Skill in operating personal vehicle for department activities and ability to maintain vehicle liability insurance
- Computer skills including Microsoft Word, Excel, and Outlook with the ability to learn new software programs
- Ability to perform routine walking, standing, bending, lifting, and stooping during the course of the day
