Care Coordinator
Job Purpose
The Care Coordinator manages the coordination and development of ongoing and new programs, including planning and organizing program activities. In addition, provides day-to-day care coordination for program patients enrolled or enrolling in programs.
Primary Duties and Responsibilities
The Care Coordinator performs a wide range of duties, including the following:
Plan the program
- Plan the delivery of the overall program(s) and its activities following the mission and the goals of the organization
- Assist in developing new initiatives to support the strategic direction of the organization
- Develop and implement long-term goals and objectives to achieve the successful outcome of programs
- Assist with the development of a program evaluation framework to assess the strengths of programs and to identify areas for improvement
Organize the program
- Ensure that program activities operate within the policies and procedures of the organization
- Ensure that program activities comply with all relevant professional standards
- Assist with the development of forms and records to document program activities
- Manage the collection and maintenance of concise records on the patients in the program for statistical purposes according to the confidentiality/privacy policy of the organization
Staff the program
- Position report to Program Director
Program management
- Coordinate care for programs by screening for eligibility and processing all primary, specialty, dental, and other referrals
- Provide case management services for enrolled patients
- Collaborate with other staff
- Assist with writing reports on the program for management and funders
- Monitor the program activities regularly and conduct an annual evaluation according to the program evaluation framework
- Report evaluation findings to organization management and recommend changes to enhance programs as appropriate
Qualifications
Education & Experience
- University degree in a related subject
- 3 to 5 years in a related field
- None
- Knowledge of program management
- Knowledge of client groups and issues related to the program area
- Word processing
- Simple accounting
- Databases
- Excel spreadsheets
- Second Language(s) encouraged

- Solve Problems: Assess problematic situations to identify causes, gather and process relevant information, generate possible solutions, make recommendations, and resolve the problem.
Working Conditions
- Care Coordinator usually works in an office environment, but the organization’s mission may sometimes take them to non-standard workplaces.
- Care Coordinator works a standard workweek but may be required to work some evenings and weekends to monitor program activities and it may require flex worktime.
