Community Outreach Nurse

Position Summary:
The Community Outreach Nurse will be a part of a community-based outreach team that responds to community needs. The role will focus on needs related to COVID-19, but specific job duties and focus will change based on need. This position will work closely with the Director of Special projects to coordinate community testing, screening and outreach events.  The Community Outreach Nurse will assist with supervising day to day operations of community testing events including all clinical aspects, scheduled staff, and volunteers. Although this position is based at KC CARE Health Center (temporarily remote), this position will be very mobile and spend the majority of their time actively working at events within the community in all weather conditions.
 
Minimum Requirements:

  • A degree from an accredited school of practical nursing is required for LPN
  • A degree from an accredited school of nursing with minimum ADN but BSN preferred.
  • Must be licensed in Missouri as either a Licensed Practical or Registered Nurse in accordance with state regulations
  • Experience in public health preferred.
  • Two (2) years’ Nursing experience
  • Basic Life Support (BLS) Certification required.
  • Ability to relate to a broad and diverse spectrum of the community.
  • Ability to work independently, under pressure in a fast-paced environment.
  • Reliable and accessible transportation along with current automobile insurance coverage
  • Ability to prioritize and perform multiple tasks.
  • Ability to understand and present verbal instructions and to exchange verbal information.
  • Ability to work in a professional manner and establish and maintain effective, courteous working relationships with patients, coworkers and others.
  • Ability to work in Windows based computer environment.
  • Ability to supervise community events including outreach staff and volunteers. 
 
Essential Functions:

Clinical Care Duties:

  • Demonstrates a strong knowledge of nursing practice and procedures.
  • Knowledge of state health regulations for public health and can provide recommendations for community testing and outreach events.
  • Work closely with the Director of Special Projects to plan, implement, and staff community testing and outreach events.
  • Supervise staff and volunteers assigned to the community events (in the absence of the Director of Special Projects).
  • Adhere to established patient screening procedures.
  • Inventory, order supplies and stock outreach vehicle(s).
  • Collect and properly store specimens in diverse environments.
  • Manage day to day community-based event operations including staffing needs, supporting fellow team members, addressing patient concerns, and troubleshooting solutions if complications arise.   
  • Maintain knowledge of current guidelines and recommendations, including vaccination information, for COVID-19 and other infectious diseases.   
  • Provide vaccinations and document as required in EHR and online registries such as ShowMeVax.
  • Communicate reactive and non-reactive test results to patients and provide referrals.
  • Assist with COVID-19 symptom screening as needed at any of the KC CARE Health Center locations.
  • Ability to work an occasional Saturday and evenings as requested.  
  • Assist with all other duties assigned.

  • Functions effectively as team leader at outreach events for Medical Assistants, outreach staff, and volunteers
  • Maintains objectivity and constructively deals with problem situations. 
  • Documents all activities completed within the electronic health record.
  • Maintains tracking in databases, excel spreadsheets and any other documentation regarding public health needs or issues as determined by administrative needs.

  • Adheres to safety, infection control and hazardous waste procedures.
  • Maintains patient confidentiality at all times in accordance with HIPAA regulations.

Physical Demands/Working Conditions:
  • Requires working outdoors in different Kansa City weather elements including, heat, cold, rain and snow.
  • Requires long periods of standing.
  • Requires ability to lift, carry, push and pull up to 30 pounds.
  • Requires ability to stoop, kneel, bend, crouch and reach.
  • Requires ability to use routine office equipment such as computer and telephone.
  • Requires ability to comprehend both oral and written communications.
  • Requires sufficient near-vision to be able to read document and computer screen.
  • OSHA Category 1: All procedures or other job-related tasks involve an inherent potential for mucous membrane or skin contact with blood, body fluids or tissues or a potential for spills or splashes of these fluids.
  • The employee is expected to travel by automobile and is exposed to changing weather conditions.
  • The employee is expected to drive to community events, outreach activities, community meetings or other agencies.
  • Requires the ability to follow and comply with proper PPE (Personal Protective Equipment) guidelines and policies and be able to wear appropriate PPE for extended periods of time throughout shift.
    • Adequate PPE and safety training will be provided and must be followed to ensure staff are able to safely and effectively protect themselves, their colleagues, and the patients.
  • May experience traumatic situations including psychiatric traumatized and deceased patients.
