Lead Community Health Worker

The Lead Community Health Worker is responsible for day-to-day coordination and the provision of services for participants enrolled in the Kansas City Healthy Start Initiative.

  • Coordinates oversight of Community Health Workers (CHWs) with Kansas City Healthy Start Initiative’s Project Director and Site Administrator
  • Convene bi-weekly meetings with the site team members for case conferencing and to plan and discuss activities/events that will recruit and engage pregnant referrals
  • Prepares staff meeting minutes for timely submission to the Program Director
  • Follows the KCHS Initiative’s protocol for care coordination and case management
  • Responsible for the annual enrollment, assignment and oversight of case management for at least 85 pregnant women, 28 dads/males, and 85 postpartum women and their infants up to 18 months of age
  • Assists with recruiting and providing referral resources to significant other males and fathers of the children enrolled in the program
  • Administers depression screens to program participants and makes appropriate referral
  • Assures referral and/or enrollment of program participants and their families in health coverage as appropriate
  • Develops a plan of care with the program participant and modifies plan when appropriate
  • Provides assistance in locating, expediting, and coordinating medical and psycho-social services which are culturally appropriate including managing immunization and well care compliance
  • Collect and maintain data and prepares monthly reports for the Evaluation Teams
  • Collaborates with internal and external resources to coordinate care, streamline referrals and maintain resources
  • Recruits (at least 1) program participants to participate in the Community Action Network



Education Requirement:
  • Bachelor’s degree in social work, nursing, or related fields
Qualification Requirements:
  • At least (1) year experience working in the maternal and child area
  • Demonstrated supervisory skills
  • Knowledge of community resources in the Kansas City area
  • Ability to work effectively with project staff and to be a team player in addition to being self motivated and bringing innovative ideas to improve quality of care
  • Capacity to maintain full confidentiality of program participant information and data
