Patient Care Specialist

POSITION SUMMARY 

The Patient Care Specialist will work to provide careful communication to patients and pharmacies regarding health information. The Specialist will engage with the Black LGBTQ+ community within the Greater Kansas City Metro Area with the aim of HIV prevention and linkage to care. This position will work to maintain a smooth flow of scheduling provider visits, recording data for collection, storage, retrieval and reporting. This position is responsible for observing and abiding by HIPAA and the HIV Confidentiality Law requirements. The Patient Care Specialist will assist the Collaborative Care Team in providing a culture of care for the Black LGBTQ+ community that BlaqOut serves. 

 

ESSENTIAL DUTIES

CARE COORDINATION:

  • Greeting incoming patients in a professional and welcoming manner

  • Responsible for all communications regarding patient health information and pharmacy

  • Facilitation of scheduling provider visits utilizing the electronic health record platform

  • Electronically record data for collection, storage, retrieval, and reporting

  • Observe and abide by HIPAA and the HIV Confidentiality Law requirements

  • Complete administrative duties that directly impact patient care 

  • Responsible for scanning and inputting all paperwork in addition to organizing documents 

  • Identify and retrieve missing documentation from patient files which will then be reported to the direct supervisor 

  • Ensuring that lab materials and physical files are organized on a regular basis to ensure a clean and tidy environment 

  • Attending weekly team meetings and meetings with important stakeholders as requested

  • Completion of all relevant training when required

  • Attending and assisting community outreach events as necessary

  • Ability to work in both independent and team environments

  • Ability to follow instructions and respond to management direction 

  • Contact and manage interactions with Patients, Medical Providers, and Pharmacies

  • Respond to Patient inquiries via an online chat feature, email, and phone

  • Help facilitate a seamless patient-to-provider interaction

  • Collaborate with other disciplines internally and externally to coordinate patient services and community needs

  • Establishes and or follows HIV prevention policies, procedures, and partnerships and schedules 

  • Linkage to care to community partners  

  • Ability to multitask and work under time constraints

  • Perform HIV risk assessments and testing as needed

MINIMUM REQUIREMENTS/PROFESSIONAL EXPERIENCE/SKILLS:

  • Must be able to demonstrate high levels of cultural competency

  • High School Diploma or GED

  • Knowledge of and comfort with LGBTQ+ Communities, HIV/AIDS prevention education, PEP, PrEP, and sexual health issues

  • Commits to BlaqOut, Inc.’s mission, vision, and core values

  • Excellent oral and written communication skills

  • Solid technological skills; especially as it relates to EHR platforms, Google Workspace, and Apple products

  • Able to obtain HIV certification within three months of employment- Employer-provided training

  • 1 year of experience with EHR systems

  • Helpful but not required:  3 years of experience or more as a Patient Care Coordinator

  • Helpful but not required:  2 years of experience with EHR systems

  • Helpful but not required:  Prior experience in HIV prevention/linkage to care

KNOWLEDGE/SKILLS/ABILITIES  AND OTHER QUALIFICATIONS

  • Patient care experience required

  • Thorough understanding of how HIV is transmitted and prevented

  • Familiarity with basic information about HIV treatment

  • Knowledge of the latest research findings on HIV prevention efforts for gay/bisexual/BMSM 

  • Familiarity with local and national epidemiological distribution of HIV disease 

  • Strong ability to communicate well both orally and in writing a must

  • Excellent administrative and organizational skills is vital

  • Demonstrate unconditional positive regard to clients and coworkers and conduct all aspects of job responsibilities with a focus on exceptional internal and external customer service

Job Type: Full Time

Salary Range: $30,000 - 49,999
