Position Description: 

Job Title:                            Case Manager, Empowerment & Homeless Prevention
 

Status:                                Exempt, Full-Time
                                               
Job Definition:                   The Empowerment & Homeless Prevention Case Manager will assist with implementation of a case management plan that will promote clients’ strengths, advance clients’ well-being, and help clients achieve their goals.  The Case Manager connects client households to mainstream services, and provide housing stability case management to assist households in obtaining or maintaining permanent housing. Salary DOQ.

Job Qualifications: 
Education:                          Bachelor’s or Master’s degree in Social Work, Human Services, or related filed preferred
 
Experience:           
              Prefer two years prior experience using Strengths-Based Case Management 
 
Current KS/MO driver’s license and proof of insurance (must submit to driving record and criminal background check).
 
Duties and Responsibilities:
 
Case Manager shall possess excellent communication skills and demonstrate patience and kindness while working under pressure and delivering quality service.
 
Case Manager will collaborate with other agencies while working with local school districts, CoC Coordinated Entry, Impact Missouri, and collaborative agencies to plan, implement, monitor, and amend individualized services while working in an efficient and effective manner. 

Case Manager will coordinate homelessness prevention financial assistance to income eligible households at-risk of homelessness in Jackson and Wyandotte Counties, including landlord-tenant relations, housing inspections, and financial assistance management.

  
Some responsibilities include: 
  • Assist clients in achieving economic stability
  • Manage client case files and documentation
  • Direct clients to appropriate resources
  • Advocate for clients
  • Provide life skills coaching
  • Utilize motivational interviewing and coaching
  • Responsible for making follow-up phone calls
  • Assist clients with monthly budgets
  • Develop and maintain relationships with other social service providers
  • Reporting and compliance duties
  • Assist clients with income supports
  • Perform housing inspections
  • Recruit and establish landlord relationships
  • Other duties as assigned
