Health Care Case Manager

Job Title: Healthcare Case Manager
Reports To: Program Director; Executive Director
Job Type: Full-Time

Summary: The Healthcare Case Manager will focus on providing Spanish/English interpretation and intensive case management services to migrant and seasonal farmworkers residing in Lafayette County, Missouri.
Essential Job Responsibilities:

  • Advocate for the basic rights and needs of migrant and seasonal farmworker clients by facilitating access to healthcare and providing meaningful interpretation services.
  • Assist clients in scheduling all types of healthcare encounters (primary care, chronic disease management, dental, children’s medical, specialty care, etc.) and accompany clients to their appointments.
  • Provide accurate and complete Spanish/English interpretation during clients’ healthcare encounters.
  • Coordinate referrals for specialty care on behalf of migrant and seasonal farmworker clients.
  • Develop basic health education interventions that are relevant to farmworkers’ lives.
  • Document case notes from clients’ healthcare encounters and keep client files organized with up-to-date case management information.
  • Organize schedules with local clinics and other service providers to facilitate healthcare access for migrant clients during special evening and weekend hours during the apple harvest season (Aug-Oct).
  • Participate in general MFAF outreach programming during the fall harvest season, including visits to migrant labor camps and weekly distribution of food and other essential items to farmworker families.
  • Assist with other MFAF programming, as needed—youth programs, educational support, emergency assistance, adult/family programs, and administrative tasks.

  • Valid U.S. driver’s license
  • Proficient in Word, Excel, PowerPoint, Gmail, and Google calendar
Position Requirements: Preferred Education and/or Experience
  • Desire to work directly with migrant farmworker clients and engage in social justice issues that affect their lives
  • Minimum of some post-secondary education; Bachelor’s degree preferred
  • Minimum of two years’ professional experience; prior case management experience preferred
  • Must be bilingual in Spanish/English (spoken and written)
  • Prefers team-oriented work environment
  • Must be available to work some evening and weekend commitments to meet the needs of farmworker clients and their families
