Position Description:
Kansas City Hospice and Palliative Care is searching for a Licensed Social Worker (LMSW) to provide in-home patient care.  The Social worker will assist patients and their families to adjust to illness and grief, resolve personal, financial and environmental issues that occur with expected death, and provide grief counseling support.

The Ideal Candidate Will Possess:
  • Master’s degree in Social Work
  • Current, unencumbered, Missouri LMSW license (dual Kansas LMSW license preferred)
  • Minimum of one year relevant Medical Social Work experience required; two to five years similar experience preferred  
  • Prior, relevant grief counseling experience
  • Prior experience conducting community home visits, preferred 
  • Reliable transportation with current auto insurance

  • Assessing patients, their families and/or primary caregivers to obtain information relevant to a psychosocial assessment including, an evaluation of the social, spiritual, financial and environmental issues that might occur with an anticipated death. 
  • Providing education regarding Advanced Directives, Durable Power of Attorney (DPOA), Do Not Resuscitate (DNR), and other issues associated with end of life care
  • Providing educational in-service training regarding the psychosocial needs of the hospice patient/family to the hospice staff, volunteers, and other agencies when needed.
  • Communicating effectively and professionally within the team environment, demonstrating respectful and appropriate language and nonverbal behavior; actively seeking to resolve customer and co-worker concerns in a timely manner.
  • Demonstrating the ability to recognize, understand, and appreciate our commitment to Diversity, Equity and Inclusion (DEI), by demonstrating social and cultural sensitivity appropriate to diverse patients, families, and staff.
