Older Adult Care Manager

Position Description: JFS Older Adult Services work to empower older adults to age in community through an array of supportive services including transportation, home maintenance and care management. This role is based in our Brookside office and provides services to clients residing in their own homes in the JFS Older Adult Service Area.  
 
We are looking for a candidate experienced in the below activities in alignment with the National Association of Social Work’s core values of service, social justice, the dignity & worth of the individual, the importance & centrality of human relationships, and integrity & competence: 

  • Provide community-based care management to older adults and/or their caregivers. 
  • In collaboration with the client, craft a client a goal plan appropriate to the needs of the person served. 
  • Implement client-centered, trauma-informed interventions with clients. 
  • Document all client contact in accordance with agency procedure and policy. 
  • Coordinate and partner with health care providers, caregivers, family members & other community programs and resources. 
  • Provide information and referral.

Requirements

Education, Knowledge, and Experience: 
  • Bachelor’s or Master’s Degree in Social Work or other relevant discipline. 
  • Experience providing client-centered, trauma-informed care management services including assessment, goal planning, and intervention & resource connection.

  • Ability to maintain client records according to agency policies and procedures regarding sensitive information and confidential situations. 
  • Working knowledge of aging resources. 
  • Ability to cultivate warm, supportive, collaborative relationships with clients and colleagues.  
  • Cultural competence and sensitivity to working with and for clients from varied racial backgrounds, with special emphasis on serving clients from racial, ethnic, and religious minority communities. 
Environmental/Physical Requirements: 
  • Ability to drive your own vehicle in Jackson County, MO & Johnson County, KS
  • Ability to carry up to 20lbs. 
  • Ability to communicate with clients by phone and via devices including a computer, tablet, and smartphone 
  • Strong familiarity with Microsoft Office Suite and associated programs.  
  • High level of comfort with internet and client database system usage. 
  • Able to comply with JFS's mandatory COVID vaccination policy.

Salary Range: $50,000 - 74,999
