Community Health Worker

The Community Health Worker is responsible for providing services for participants enrolled in the Kansas City Healthy Start Initiative at Nurture KC. 

Role & Responsibilities:

  • Enroll and manage a caseload of at least 40 program participants assigned by supervisors.
  • Actively recruit participants to build the expected caseload through outreach.
  • Provide on-going care coordination to pregnant and postpartum women and their infants up to 18 months of age.
  • Provide case management and referral to male involved partners of the children enrolled in the program.
  • Administer depression screening to program participants and make appropriate referrals.
  • Assure referral and/or enrollment of program participants and their families in health coverage under ACA.
  • Develop a plan of care with the program participant and modify the plan when appropriate.
  • Provide assistances in locating, expediting, and coordinating medical and psycho-social services which are culturally appropriate.
  • Attend all required meetings and scheduled training.
  • Attend community events that might include some nights and weekends.
  • Collect and maintain timely data for monthly reports for the Evaluation Team.
  • Assist with the recruitment of program participants to participate in the Community Consortium.
  • Adheres to the guidelines of all federal, state, local and foundation grants.
  • Other duties as assigned.
 Education Requirement:
  • Bachelor’s degree in social work, health sciences, or related field.
 Qualification Requirements:
  • At least (1) year experience working in the maternal and child area (preferred).
  • Familiarity with the neighborhood services and resources in the program service area.
  • Experience with grassroots organizations and coalescing neighborhoods and their residents.
  • Capacity to maintain full confidentiality of program participant information and data.
  • Ability to work effectively with project staff and to be a team player.
  • Self-directed with the ability to complete all tasks on schedule.
  • Proficiency with Microsoft Suite.

The salary for this position is $44,500 annually.
