Community Navigator - PT
Job Title: Community Navigator

Location: South Kansas City
Community Navigator (Part-Time)
Case Manager (Part-Time)
Total Weekly Hours: 20
- Client Direct Services: 10 hours (50%)
- Documentation: 5 hours (25%)
- Events, Meetings, etc.: 5 hours (25%)
The Community Navigator(s) at the Community Assistance Council provides comprehensive navigation and case management services to support the social-emotional well-being, physical, cognitive, and social development of clients. This role focuses on enhancing cultural competencies, career and vocational skills, economic mobility, and mental, physical, and behavioral health for adult clients and families. The Community Navigator is responsible for conducting needs assessments, connecting clients to vital resources, facilitating programs for community integration, and maintaining accurate records.
- Client Direct Services
- Provide individual or group support, outreach, and case management services.
- Complete a minimum of 5-7 client interactions per week (approx. 1.5-2 hours per interaction, including preparation).
- Log services in real time or within 24 hours of service delivery.
- Documentation
- Maintain up-to-date and accurate client records, including case notes, assessments, care plans, and progress reports.
- Allocate an average of 1 hour per client interaction for documentation.
- Use available templates and systems to streamline documentation processes.
- Events, Meetings, etc.
- Attend organizational meetings, professional development sessions, and community events as assigned.
- Participate in a minimum of 1-2 events or meetings per week to meet this requirement.
Qualifications:
- Education and Certification:
- Certification as a Community Health Worker is required.
- Bachelor’s degree in Social Work, Public Health, or a related field is preferred.
- Experience:
- 3-5 years of case management experience, particularly in nonprofit settings.
- 3-5 years of experience in resource allocation and working with diverse populations.
- Skills and Competencies:
- Bilingual proficiency preferred.
- Strong communication skills to engage with clients and community partners effectively.
- Strong analytical and problem-solving abilities to address complex client needs.
- Proficiency in Microsoft Office Suite and client management systems.
- Professional Requirements:
- Professional liability insurance is required.
- Demonstrated knowledge of nonprofit systems and community resources.
- Experience working with culturally diverse communities and promoting inclusion.
- Familiarity with South Kansas City communities and their specific needs.
This position may require occasional evening or weekend work to meet client needs and participate in community events. Local travel is expected.
